On your first visit, please bring the following information:

A list of medications you are currently taking
Any current medical concern or condition
Dental insurance, if you are covered and paying through insurance
Completed new patient forms (found below)
Please note that all patients under 18 must be accompanied by an adult.

Because oral health is so important, we do everything we can to remove financial barriers and help the greatest number of patients possible. We accept most insurance providers and CareCredit and Sunbit Finance as alternatives for those without insurance.

Dr. Paracha, a distinguished graduate of the University of Pennsylvania, is celebrated for earning his DMD with honors and receiving the American Academy of Oral and Maxillofacial Radiology Achievement Award. He is an esteemed educator, having taught at both the University of Pennsylvania and the University of Detroit Mercy. Specializing in Cosmetic Dentistry, Implant Prostheses, and more, Dr. Paracha is dedicated to building long-term relationships with his patients by providing compassionate, top-quality care.

Dr. Siddiqi is a distinguished Oral Medicine expert with advanced degrees from the University of Washington and an extensive teaching background.
